Summary
The RN Clinical Supervisor coordinates and supervises comprehensive home care services, and is responsible for accurate and complete documentation of the clinical records in accordance with rules and regulations. The RN supports the core values of Haven Home Health: dignity, integrity, compassion, individuality, coordination, and excellence.
Responsibilities of the RN include, but are not limited to:
Coordinates the provision of the care management of comprehensive home care services in the home care setting in accordance with rules and regulations.
Coordinates day-to-day clinical operations for assigned team.
Receives referrals from agencies/referral sources and assigns referral to appropriate patient care staff.
Ensures that total referral information is complete, appropriate for home care services, and has insurance/financial coverage.
Ensures compliance with patient services requirements, the Conditions of Participation, including OASIS requirements, accrediting standards, and agency policies and procedures.
Oversees the daily assignment of patient care visits, approves all schedule changes. Monitors clinical caseload/visits.
Oversees the accurate and timely completion of all documentation and required forms.
Assists in quarterly record reviews as assigned by the QA manager for compliance with the conditions of participation including: plans of care (485), admission OASIS, subsequent OASIS, and recertification, therapy reassessment, and face to face requirements. Communicates findings and recommendations to the QA manager.
Reviews and approves discharge plans with the clinician prior to the patient's discharge.
Identifies team staffing needs and seeks approval for additional staff, when necessary.
Ensures appropriate visit authorization, utilization, and supply management.
Assures quality care of services and maintenance of standards of practice for staff.
Provides in-home supervision of licensed/non-licensed staff during orientation, and at least annually, to evaluate quality and effectiveness of care provided.
Performs written job performance evaluation at the end of ninety days, and annually thereafter.
Participates in annual review and on-going policy and procedure development. Ensures staff education and compliance with policies, procedures, and changes.
Receives patient information/orders from physician, documents, and transcribes physician orders and communicates pertinent information to professional staff.
Other duties as assigned.
Determines eligibility for services related to home bound status and need for skilled services.
Develops and implements an individualized plan of care in consultation with the patient's physician and other agency personnel as appropriate.
Regularly re-evaluates the patient's nursing needs and revises the plan of care as necessary.
Provides direct nursing care procedures to patients.
Completes review and reconciliation of medications, pain management, and evaluation of risk status.
Furnishes services requiring substantial and specialized nursing skill.
Initiates appropriate preventive and rehabilitative nursing procedures.
Obtains specimens as ordered by the physician, and submits them to the laboratory.
Teaches patients, family members, and/or significant others health care self-management skills, accurately assess patients' physical and emotional status, and preforms services in compliance with physician orders.
Counsels the patient and family in meeting nursing and related needs.
Maintain confidentiality of all pertinent patient and employee information to assure their privacy is protected.
Ensures that arrangements for equipment and other necessary items and services are available.
Participates in team meetings/care conferences and communicates with other clinicians involved in care.
Prepares clinical and progress notes, coordinates services, informs primary physician of changes in the patient's condition and needs when necessary and at least every 60 days.
Instructs, supervises, and evaluates home health aide care provided at least every 14 days.
Manage and supervise LPNs performance in implementing nursing services.
May teach and train other nursing personnel.
Attends in-services and department meetings as required.
Participates in the agency's on-call system as necessary.
Participates in clinical outcomes monitoring, follow up, and agency performance improvement initiatives.
May occasionally work with office machines, such as computers, copy machine, and telephones.
Support and uphold all federal state, and local regulatory laws and standards.
Support and uphold Haven Home Health and Therapy policies and procedures.
Makes the initial evaluation visit except in cases where the physician has ordered therapy services only.
Conducts comprehensive OASIS initial nursing assessments including physical, emotional, and mental needs.

Qualifications, Knowledge, Skills, and Abilities

Current Missouri Registered Nurse license in good standing
Minimum of one year experience as a RN Clinical Supervisor within the last three years.
Excellent organizational and time management skills
Excellent communication skills with multidisciplinary team of clinicians, clerical staff, and management
Flexibility in work schedule to accommodate patient needs, including new admissions.
Strong clinical judgment, assessment, communication, and leadership skills
Proficient in computer skills and technology to complete documentation of clinical record
Must have dependable transportation and current driver's license
Will work weekends and holidays as needed
OASIS-C and laptop experience

Additional recommended qualifications are:
BSN
Certification in a specialty area by a national organization such as American Nurses Credentialing Center (ANCC).

Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
